Case: Two Vehicle Fatal Crash on CTY TK H at Forest View Rd

On 04/08/2026 at 4:19pm, the Washington County Sheriff’s Office Communication Center began receiving multiple 911 calls reporting a two-vehicle crash on CTY TK H at Forest View Rd in the Town of Farmington. A caller reported that the driver of a gray SUV was not responsive. Deputies, along with Boltonville Fire and First responders, and West Bend Rescue were subsequently dispatched to the scene.
Upon arrival of first deputies on scene, the driver and sole occupant of the gray SUV was found to be unresponsive. Deputies and first responders attempted life saving measures on the driver but were unsuccessful. The driver of the blue SUV was conscious and alert.
The initial investigation and witness statements showed that the blue SUV was westbound on CTY TK H approaching the intersection of Forest View Rd. The gray SUV was eastbound on CTY TK H and attempted to turn northbound on Forest View Rd in front of the blue SUV, and was hit on the passenger side.
The Washington County Sheriff’s Office Crash Reconstruction Team responded to complete additional scene documentation. CTY TK H and Forest View Rd were closed for approximately three and a half hours for the emergency response, documentation and cleanup efforts. The incident is still under investigation at this time.
